Re-certification of civil status acts

Required documents

  • Application for the relevant re-certification:
  • Re-certificate of birth (download)
  • Re-certificate of death (download)
  • Re-certificate of marriage (download)
  • Re-certificate of divorce (download)
  • Re-certificate of determination of paternity (download)
  • Re-certificate of change of name, patronymic and surname (download)
  • Re-certificate of adoption (download)
  • Copy of passport or identification card (şəxsiyyət vəsiqəsi) of the applicant;
  • Documents confirming the kinship (birth, marriage certificates) for obtaining a re-certificate of death.

Fee

  • 10 USD. Only Money Orders are accepted. Please, make the payment to the order of the Embassy of Azerbaijan;
  • 2 AZN state fee must be paid to the address below and the receipt must be submitted to the Consular Section of the Embassy along with other documents;

Who can apply

  • The person for himself/herself
  • In the case of minors, their parents, adoptive parents, guardians or custodial authorities; education, treatment and social protection bodies
  • For deceased person - their legal heirs
  • Authorized persons on the basis of a notarized power of attorney

How to apply

  • Only complete applications will be accepted.
  • Please, note that the Consular section of the Embassy accepts applications for re-certification of civil status acts via mail only. The request must include a pre-paid return envelope (Express Mail, FedEx, UPS or DHL). Envelope will be used to ship the issued re-certificate back to you. The Embassy of Azerbaijan does not assume any responsibility for the loss postal delivery. As the Embassy does not keep tracking numbers of your mail, you are advised to keep a record of it in order to track your mail returning to you.
